10/27/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. During morning observations, the client exhibited difficulty with controlling impulsive behavior by not staying inside his classroom. During midday observations, the client still exhibited difficulty with controlling impulsive behavior by not staying in his designated lunch area. During afternoon observations, the client was not able to communicate with authority in an appropriate manner when asked to stay in his assigned bus area. QMHP-C greeted the client upon arrival to the school setting. During morning observations, QMHP-C redirected the client to his seat. QMHP-C engaged the client in discussion reiterating the importance of controlling impulsive behavior by staying inside of his classroom. During midday observations, QMHP-C redirected the client to his lunch space. QMHP-C praised the client for the ability to regain track in an effort to achieve his goal. QMHP-C engaged the client in discussion surrounding the importance of staying in his designated lunch area. During afternoon observations, QMHP-C redirected the client to maintain focus. QMHP-C reiterated the importance of achieving all objectives throughout the day. QMHP-C encouraged the client to continue working towards achieving his goal. The client responded well towards TDT services as evidenced by participation and engagement. During morning observations, the client was unreceptive to redirection by running through the hallway. The client was then able to calm down with prompting and redirection. The client agreed to control impulsive behavior by going back in his classroom upon prompting. During midday observations, the client was receptive to redirection and agreed to stay in his assigned lunch area. The client appeared to struggle with communicating his feelings as evidenced by yelling during interactions with peers. When inquired why he was utilizing a loud tone, the client informed that he was unsure. During afternoon observations, the client was unreceptive to redirection by crying and screaming inappropriate language. The client was then able to calm down and agreed to work toward achieving all of his target goals. The client also agreed to continue working towards achieving his goal. QMHP-C will continue to redirect and prompt the client to control his impulse as evidenced by staying in his seat. QMHP-C will continue to encourage the client to communicate/respond to authority figures in an appropriate manner by verbalizing his thoughts and feelings daily. QMHP-C will continue to encourage the client to follow directives from authority figures within 3 prompts. QMHP-C will continue to provide support to the client within the school setting in an effort to maintain in school learning.
